29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) On or about January 15, 2014, kitchen employees were using products that contain corrosive chemicals including, but not limited to, GreaseCutter Plus and Oasis 137 Orange Force; products that contain chemicals that are a slight to severe eye irritants including, but not limited to, Apex Metal Protection, Apex Presoak, Wash N Walk No-Rinse Floor Cleaner, Oasis 255 SF, Apex Manual Detergent, Kool Klene and Keystone Apex Rinse Additive. Employees were not using chemical goggles and/or face shields. (b) On or about January 15, 2014, Laundry/Maintenance Closet employees were using products to perform their duties that contained corrosive chemicals including, but not limited to, Chemtron Stain Remover and Diversey Power-foam Bravo Heavy Duty Stripper for Spot Buildups. Employees were only provided with plastic glasses.

No eyewash station was in the closet where the product was being dispensed. (Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
