29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a)  Jobsite located at 4408 Augusta Road Building #1, Savannah GA - On or about January 15, 2014, employees were using extension cords, which were missing a ground prong, including the orange extension cord with a black stripe and a yellow extension cord that were used to supply power to a Makita 5007MG circular saw, exposing employees to an electrical hazard.      b) Jobsite located at 4408 Augusta Road Building #1, Savannah GA - On or about January 15, 2014, a portable air compressor Makita MAC5200 power cord was being used without a ground pin, exposing the employee to an electrical hazard. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a)  Jobsite located at 4408 Augusta Road Building #1, Savannah GA - On or about January 15, 2014,  two extension cords used to supply power to electric hand tools were without strain-relief in that the outside protective sheath was pulled from the neck, exposing employees to an electrical hazard. 29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a)  Jobsite located at 4408 Augusta Road Building #1, Savannah GA - On or about January 15, 2014, employees were using a personal fall arrest system (PFAS), consisting of a full body harness, with a 6-foot shock absorbing lanyard attached.  The 6-foot lanyards, which were wrapped around the top scaffold ledger would have allowed the employees to impact the lower surface in the event of a free fall, exposing the employees to an 11-foot fall hazard.      ABATEMENT CERTIFICATION IS NOT REQUIRED
