29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.    On or about January 15, 2014, the employer did not ensure personal protective equipment, such as hard hats were being worn by employees while working underneath other employees on higher levels of the tubular welded frame scaffold.  Employees were exposed to struck by hazards.

On or about January 15, 2014, the employer did not ensure employees performing masonry activities such as cutting brick with a DeWalt saw were wearing eye and face protection exposing employees to struck-by hazards and eye injuries.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.    On or about January 15, 2014, the employer did not ensure the following equipment was safe and free from recognized hazards such as exposing employees to electrical shock hazards:  a)  The yellow electrical extension cord used to energize the Baldor saw had the outer sheathing pulled away at both ends of the cord from the strain relief devices, and  b)  The green electrical extension cord used to energized a hand saw had the outer sheathing pulled away from the strain relief at the "T" pole located on the northwest corner of the job site.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On or about January 15, 2014, the employer did not ensure that a ladder was provided to access or egress the scaffold, exposing employees to fall hazards.

On or about January 15, 2014, the employer did not ensure that all levels of the tubular welded frame scaffold were fully decked, exposing employees to fall hazards.  The Aurelio Ayala was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 314273269, citation number 1, item number 1 and was affirmed as a final order on 04/29/2010, with respect to a workplace located at 9036 Quarry Ridge Trail, Fort Worth, TX 76244.
