29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, stairs, in and around buildings or other structures. (a) Nichols Design Group, P.C. @ 900 So. Franklin Street, Denver, CO 80209: On and before January 15th, 2014, as part of the injury/accident prevention responsibilities, the controlling employer did not ensure the work areas throughout the site, around the unguarded window wells, and in front of the two- story home under construction where employee were engaged in framing operations, was kept clear of scrap lumber and 2x4 boards having nails sticking out. This condition exposed the subcontractor employees to puncture hazards and tripping/falling injuries.

This condition exposed the employee to the hazard of falling 7 feet to the lower level.

Franklin Street, Denver, CO 80209: On and before January, 15th, 2014, as part of the injury/accident prevention responsibilities, the controlling employer did not ensure that fall protection systems were provided to employees when sheathing the 5 in 12 pitch roof on a two-story residential home under construction. This condition exposed the employees to the hazard of falling up to 20 feet eave to ground. (b)Nichols Design Group, P.C. @ 900 So. Franklin Street, Denver, CO 80209: On and before January 15th, 2014, as part of the injury/accident prevention responsibilities, the controlling employer did not ensure that fall protection systems were provided to subcontractor employees while working from a single 2 x 12 wood plank performing framing activities on the two-story residential home under construction. This condition exposed the employees to the hazard of falling up to 9 feet to the lower level. (c)Nichols Design Group, P.C. @ 900 So. Franklin Street, Denver, CO 80209: On and before January 15th, 2014, as part of the injury/accident prevention responsibilities, the controlling employer did not ensure each subcontractor employee on a walking/working surface having an unprotected side or edge which was which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. Subcontractor employees were engaged in framing operations on the 1st floor having near an unprotected stairway rough opening for the basement of the two-story residential home under construction. This condition exposed the employee to the hazard of falling 10 feet to the lower level.
