29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:  1) Welding Area -The employer allowed employees to perform MIG welding activities behind a torn welding screen causing adjacent welders to be potentially exposed to welding arc light.  2) Drilling Area - The employer failed to place a welding screen between the welder and adjacent employees drilling, employees were exposed to welding arc light.  3) Tube Mill Stacking Area - The employer failed to ensure that welding screens were placed between the welder and adjacent employees, employees were exposed to welding arc light.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible.  1) Saw Department - Employees voluntarily using a N95 filtering facepiece (Serial # TC-84-A-3714) were not provided a copy of Appendix D of the respirator standard.   The North American Container Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(i), which was contained in OSHA inspection number 312778392, citation number 2, item number 1 and was affirmed as a final order on August 17, 2010, with respect to a workplace located at 270 Johnson Lake Road, SE, Adairsville, Georgia 30103.
