29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  JOB SITE: 1846 Yellow Rock Road, De Soto, MO 63020, at this work site, located on the sleeving area, on the north wall, the passage along an exit access leading outside was mistaken for an exit door was not marked "Not an Exit".  The passage along an exit access led to a dead end without direct access to the designated assembly area.

JOB SITE: 1846 Yellow Rock Road, De Soto, MO 63020, On and before 1/17/2014, the employer did not provide updated Hazard Communication Training to include new labeling elements and new safety data sheet formats.  Employees were not aware of changes to safety data sheets to include, but not limited to, the use of pictograms and hazard statements.
