29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:   a) On or about 1/17/14, Test Bay:  End of Amtrak vehicle.  No midrail was provided at the car end opening.  Employees exposed to falls of eight feet, six inches.      b) On or about 1/17/14, Test Bay:  Side of Amtrak vehicle.  No midrail was provided at the car side opening.  Employees exposed to falls of four feet, ten inches.     c) On or about 1/17/14, final Assemble, Building #4:  No guardrail system was provided at the end of a rail car, at the access point. Employees exposed to falls of over four feet due to the access stairs configuaration.   Abatement certification is required for these items.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) On or about 1/17/14, Test Bay:  Inside a Houston car, a heater had exposed, energized 480 volt terminals.  b) On or about 1/17/14, Test Bay:  Inside a Houston car, empty light sockets had exposed, energized 120 volt parts.    Abatement certification is required for these items.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):   a) On or before 1/17/14, Test Bay:  Employees were working on and near 480 Volt AC circuits that were not locked out.  A circuit was re energized while employees were exposed.   b) On or about 1/17/14, Test Bay:  Employees were working on and near 650 Volt DC circuits without each employee applying a lock out device.   Abatement documentation is required for this item.
