Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 1/14/2014, an employee from MSS Roofing LLC was working on the roof of a residential building located at 3960 Highway Z, New Melle, MO. He was not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employee was observed on the roof of the building without fall protection, exposing him to fall to the ground below.
