29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments, the approximate weight of the truck and attachment combination, and the capacity of the truck and attachment combination at maximum elevation with load laterally centered:  Frozen Food, Battery Charging Room - The Crown model RC5545-40 lift truck, serial number 1A343231, was not provided with a capacity data plate corresponding to the layer picker attachment manufactured by Tygard Machine & Manufacturing Company.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    1987 Freezer/Dairy Building, North end of Condenser #4 - Employees walking on the designated walkway were exposed to a fall hazard from the roof's unprotected edge to the ground below.    Hannaford Brothers Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(a), which was contained in OSHA inspection number 315298265, citation number 1, item number 1 and was affirmed as a final order on 7/22/11, with respect to a workplace located at 22 Fitchburg Road, Ayer, Massachusetts.

The documentation did not identify the date of the inpsection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test:    1987 Freezer/Dairy Anhydrous Ammonia Refrigeration System - The last documented calibration of Honeywell electrochemical gas sensors was 12/20/12.  Honeywell performed that calibration and created that document which specified a calibration due date of 6/20/13.
