29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about January 16, 2014, on the east side of the construction site, an employee installing a canopy on a commercial building was not tied off, exposing the employees to a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about January 16, 2014, on the east side of the construction site, employees installing a canopy on a commercial building utilized an extension ladder that did not extend 3 feet above the upper landing, exposing the employees to a fall hazard of approximately 10 feet.
