29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.    On or about January 16, 2014, on the west side of the construction site, at least one employee working from a Genie Z-45/255 boom lift to access the upper working area was not protected from falling more than 25 feet by the use of a full body harness and lanyard attached to the basket.
