29 CFR 1910.26(c)(2)(vii): Ladders having defects were not marked and taken out of service until repaired.   On or about January 17, 2014 and times prior thereto, the employer did not ensure ladders with defects were removed from service. Employees who worked in the Urea Batch Mixing Department used a 16 foot Werner extension ladder with a broken rung exposing employees to fall hazards.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  On or about January 17, 2014, and times prior thereto, employees were not medically fit tested or trained in the proper use and care of air purifying respirators provided by the employer for their use.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  On or about January 17, 2014, and times prior thereto, the employer did not ensure all pull boxes, and junction boxes were provided with covers exposing employees to electrical shock hazards:   1.  The electrical junction box located on the wall by the Roytronic Metering Pump Anti Scalant station did not have a cover.  2.  Conduit located by the door on the east side of the building that exited to rail dump pit was not provided with a cover.  3.  An electrical box located on the west wall of the production area where employees turn on the lighting of   the production area did not have a cover.
