Employer could not secure source consent for blood analyses. The employer did not document the lack of consent to test. The condition was noted on or about 01/21/2014. The condition was noted on or about 01/21/2014. 29 CFR 1910.1030(g)(2)(ii): The employer did not ensure that training provided to employees with occupational exposure met the requirements of 29 CFR 1910.1030(g)(2)(ii)(A) through (g)(2)(ii)(B):      a) The employer, Bensonhurst Center, did not ensure that the information and training provided to the employees with occupational exposure to blood or other potentially infectious materials met the requirements described in the Bloodborne Pathogens standard (BBP). Employees demonstrated lack of knowledge, and the training program missed some of the required elements, such as the regulatory text of the Bloodborne pathogens standard (BBP) and information on the hepatitis B vaccination. The condition was noted on or about 01/21/2014. At the time of the opening conference the employer did not have a sharps injury log for the recording of percutaneous injuries from contaminated sharps. The condition was noted on or about 01/21/2014.
