29 CFR 1926.1101(f)(2)(i): The employer failed to ensure that the exposure assessment was conducted in time to comply with requirements which are triggered by the exposure data or the lack of a negative exposure assessment: a) YWCA 32 Cottage Street, Lockport, NY - On or about 1/21/14, the employer did not conduct a negative exposure assessment. Personal samples were collected on 12/28/13 and 1/2/14 but were not analyzed in time to comply with any requirements of the standard which would be triggered by the exposure data. The samples were not analyzed until 1/24/14. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1101(p)(1): Mandatory Appendix A to this section was not adhered to in that the OSHA Reference Sampling Method was not followed: a) YWCA 32 Cottage Street, Lockport, NY - On or about 12/28/13-1/2/14, four air samples (Lab sample #1618-1621) were submitted for analysis by OSHA method ID-160 and there were no field blanks included. (The OSHA Reference Method requires a minimum of 2-field blanks or 10% of the number of samples, whichever is greater). ABATEMENT CERTIFICATION REQUIRED
