29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee. At this job site, employees tasked with applying a coating onto the HVAC duct work wrapped a steel alloy chain on the suspension frame holding up the HVAC system, then attached the lanyard of their harness to it to create a vertical lifeline. There was no verification that this system created an adequate anchorage point for fall protection.
