29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On January 22, 2014, at the above addressed site, an employee was exposed to a fall hazard of approximately 20 feet 6 inches while using an extension ladder which was not extended at least 3 feet above the upper landing surface.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On January 22, 2014, at the above addressed site, an employee was exposed to a fall hazard of approximately 20 feet 6 inches while using an extension ladder set up in an active construction site that was not secured to prevent dental displacement.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    On January 22, 2014, at the above addressed site, employees using ladders to perform their work were not provided with a training program for them to recognize the hazards associated with ladders.    PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
