29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: On or about January 22, 2014, the employer did not assure employees who are exposed to electrical hazards while performing work on or near exposed energized parts were trained in electrical safety-related work practices. A maintenance employee was exposed to electrical hazards in that safe work practices were not employed to prevent electrical shock or other injuries when performing electrical tests. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: On or about January 22, 2014, the employer did not develop and implement electrical safety related work practices which cover employees exposed to live electrical conductors while performing electrical work. Maintenance personnel responsible for troubleshooting and/or performing electrical tests were exposed to electrical hazards in that safe work practices were not employed to prevent electrical shock and or other injuries. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: On or about January 22, 2014, the employer did not assure that employees who troubleshoot electrical equipment are provided with and use personal protective equipment to protect from electrical hazards. A maintenance employee, using a multi-meter to troubleshoot an air compressor's 215 electrical system, was exposed to electrical shock and arc flash hazards in that insulated gloves with leather gloves, safety glasses, and a hard hat was not worn during the troubleshooting activities. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: On or about January 22, 2014, the employer did not have a written hazard communication program that informed employees of chemical hazards such as Zep's commercial shower tub and tile cleaner (sulfamic acid, hydroxyacetic acid), Xtraction II carpet cleaner (2-Butoxyethanol), and Chemcor's lemon disinfectant (didecyldimethyl ammonium chloride) which may lead to respiratory irritation, skin and eye irritation and burns.
