29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; Location - 37 Grove Street, Lexington, MA: On or about 1/21/2014, an employee working from a ladder jack scaffold that was greater than ten feet above a lower level was not protected from falling to the ground below.
