29 CFR 1910.134(d)(1)(iii):  The employer did not evaluate the respiratory hazard(s) in the workplace including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminants chemical state and physical form:       (a)   Grand County Roofing & Sheet Metal dba The Roofing Company, 28 Ten Mile Drive, Granby, CO 80446: On and before January 16, 2013, the employer did not conduct air monitoring to determine employees exposure to the air contaminants associated with Patina application performed by employees.  This condition exposed employees to respiratory hazards.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:     (a)    Grand County Roofing & Sheet Metal dba The Roofing Company, 28 Ten Mile Drive, Granby, CO 80446: On or before January 16, 2014, the employer did not provide employees with adequate hand protection while using Oakite Low Heat Cleaner 3. The employer was providing employees with latex gloves, where the Safety Data Sheet states "If potential for skin contact exits, wear neoprene or other chemical resistant gloves". This condition exposes employees to skin burns.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(h)(1):  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets     (a)   Grand County Roofing & Sheet Metal dba The Roofing Company, 28 Ten Mile Drive, Granby, CO 80446: On and before January 16, 2014, the employer did not provide employees with hazardous chemical training. The employer did not provide employees with training on chemicals such as but not limited to those used in the patina process. The employer maintained some material data sheets in electronic form, this information was not readily available to employees.
