29 CFR 1926.453(b)(2)(v): Each employee working from an aerial lift was not protected from falling by wearing a harness with a lanyard attached to the boom or basket: In the building entrance, East side, on or about January 22, 2014, and at times prior thereto an employee welding metal beams from an aerial lift was exposed to a fall greater than 6 feet, without using personal fall protection.
