29 CFR 1910.219(b)(1): Flywheel(s) with parts seven feet or less above floor(s) or platform(s) were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv): a) On the Wilson Super Double Drum Pull Spudder, operating at various locations in the oil fields, the guard on the rotating drive mechanism was not fully enclosed. The guard beside the operators station had a five inch by five inch opening in the wire mesh guarding and the solid plate guarding on the bottom had an opening 12 inches wide.
