29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a)  On or about January 22, 2013, employees were exposed to chemicals such as, but not limited to, the Wheel Ease, which contains Hydrofluoric and Sulfuric acid.  Hyrdofluoric and Sulfuric acid causes severe eye irritation, swelling of the eyelids, watering, redness, severe burns to the gastrointestinal track.  The employer did not develop and implement procedures for training employees on hazardous chemicals and labels and other forms of warning in the workplace.    Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a) On or about Janaury 22, 2014, and at times prior, the employer did not ensure employees were protected from electrical shock in the Automotive Service area due to inadequate strain relief in the following areas;  (1) One 20 amp electrical plug, located in bay 6 attached to the hydraulics of an Automotive lift, was not equipped with strain relief.  (2) One 20 amp electrical plug, located in bay 7 attached to the hydraulics of an Automtoive lift, was not equipped with strain relief.   (3) One 20 amp electrical plug, located in bay 8 attached to the hydraulics of an Automotive lift, was not equipped with strain relief.
