29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Lodge and Shipley mechanical press brake located in Fabrication shop; On or about January 23, 2014 and at times prior thereto, the employer did not develop written machine specific lockout tagout procedures to ensure that hazardous energies present on the press brake (including electrical and pneumatic) were controlled if maintenance needed to be performed on the equipment.
