29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  (a) 1505 Orrtanna Road, Orrtanna, Pennsylvania - The employer provide portable fire extinguishers for employees to use to fight incipient stage fires, and employees used portable fire extinguishers to fight a fire on the equipment trailer of a work truck on January 21, 2014.  However, the employer had not provided initial education to one employee or annual education to another employee on fire extinguisher use and the hazards of incipient stage fire fighting, exposing employees to the hazard of injurious contact with smoke or other burning/smoldering materials.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
