29 CFR 1910.146(f)(10): The entry permit that documented compliance and authorized entry to a permit space did not identify the results of initial and periodic tests performed under 29 CFR 1910.146(d)(5), accompanied by the names or initials of the tester: At Frontier Cooperative Company: The employer failed to ensure that the authorized entry permit used to document the entry into permit required confined spaces listed the initial and periodic atmospheric testing results with the names of those conducting the testing. During the inspection the company provided copies of cancelled PRCS entry permits; Section C on the permits is not filled in with initial and periodic atmospheric testing results taken prior and during entry operations as required by 29CFR 1910.146(d)(5) nor did the employer have certification data from previous entries that would substantiate why sampling was not performed for the permits provided which is required by 29CFR 1910.146(c)(7)(iii).

The permits signed by the location manager did not have any information pertaining to rescue services.

As indicated by the companys cancelled entry permits; the employer, on more than one occasion listed an employee as an attendant, who had not been trained in accordance with the requirements of 29 CFR 1910.146(i).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Next to the drag leg inside the concrete grain elevator located at Frontier Cooperative Company 351 Cottonwood Street, North Bend, NE: The employer failed to ensure that electrical equipment was used in permissible applications. During the inspection an electrical power strip was being used inside of a grain elevator to energize a sump pump. The power strip was not rated for use in combustible environments such as the grain elevator.
