On or about January 23, 2014 at a jobsite located at 8415 Gatehouse Way, Fort Smith, AR , employees performing work while standing on the top plate were not protected from falls, exposing them to the hazard of falls to the ground or concrete slab. b. On or about January 23, 2014 at a jobsite located at 8415 Gatehouse Way, Fort Smith, AR, employees performing work while standing on a header were not protected from falls, exposing them to the hazard of falls to the concrete slab.
