29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a. At the site - an employee was exposed to a fall hazard, in that, the fall protection was used improperly while operating a truck mounted aerial lift. The shock absorbing lanyard was attached to the side d-ring instead of the d-ring located on the back of the harness. Violation observed on or about 01/24/14.
