29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  At the time of the inspection, the following machines were not guarded to prevent exposure to hazards;  1. The VTC-20C MAZAK 7-238 work center was setup to allow an operator to place their hands into a point of operation while operating a hydraulic clamp and an amputation injury occurred 11/27/13. 2. The "mini" die cast machines were operated during the inspection without barrier guards to prevent the operators from placing body parts into the point of operation. 3. The Clausing-Colchester metal cutting lathe in the die shop did not have a chuck guard to protect employees from the rotating chuck.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:  At the time of the inspection, the employer had no documentation that authorized employees who locked out equipment such as die casting machines received annual reviews of the lockout procedures.
