On or about January 23, 2014, and at times prior thereto, employee(s) were exposed to an amputation hazard, from a drill press operated without a guard at the point of operation where they drilled holes of various sizes into decorative metal pieces of aluminum, brass and iron. b) South Bend Precision Lathe, 9-inch, Model A, Catalog No. 444A. On or about January 23, 2014, and at times prior thereto, employee(s) were exposed to an amputation hazard, from a lathe operated without a guard at the point of operation where they cut designs into rods of aluminum, brass and iron. c) Bridgeport Mill, Serial No. 0188892. On or about January 23, 2014, and at times prior thereto, employee(s) were exposed to an amputation hazard, from a mill operated without a guard at the point of operation where they drilled and bored shapes into sheets of aluminum, brass and iron.

Work rests was not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: Warehouse, near center of shop. Black and Decker 6-inch bench grinder, Model Type A, Serial No. 12473. On or about January 23, 2014, and at times prior thereto, employees were exposed to a struck-by hazard, as the opening between the abrasive wheel and work rest was greater than 1/8 inch, while deburring metal parts.
