29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 494 Walter Hand Jr. Road, Ozark AL; On or about January 24, 2013 and at times prior thereto the employer exposed employees to worksite hazards in that the employer failed to instruct his employees in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to their work environment to control or eliminate any hazard(s) or other exposure to illness or injuries. Employees had not been provided fall protection training and were allowed to work on a roof that was 8 feet above the ground without fall protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 494 Walter Hand Jr. Drive, Ozark AL: On or about January 24, 2014 the employer exposed employees to fall hazards in that employees were allowed to work on a roof that was 8 feet above the ground and the roof was sloped 10 on 12 without being protected from falling by a guardrail system with toeboards, safety net systems, or personal fall arrest systems.
