29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph:  (a) 9 Mohawk Dr Leominster, MA: On or about October 29, 2013, an untrained  (temporary) employee was observed operating a powered industrial truck in this facility, which is not in accordance with this subpart.

29 CFR 1910.252(b)(2)(iii):  Workers or other persons adjacent to the welding areas were not protected from the arc welding rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles:  (a) 9 Mohawk Dr Leominster, MA: On or about October 29, 2013, no less that three employees were welding in this facility, and their area was not fully screened off so that other employees in this facility were not shielded form the arc welding rays, which is not in accordance with this subpart.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) 9 Mohawk Dr Leominster, MA: On or about October 29, 2013, an Amada PEGA 357 press was not guarded to prevent the operator by being struck by the sliding table during the programmed production cycle, which is not in accordance with this subpart:  New England Wire Products Corp was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212 (a) (1), which was contained in OSHA inspection number 310630942, citation number 1, item number  3 and was affirmed as a final order on August 12, 2009 with respect to a workplace located at 307 Airport Rd Fitchburg, MA.
