29 CFR 1915.74(a)(1): The employer permitted employees to board or leave a vessel not using a substantial straight ladder that was adequately secured against shifting or slipping when a gangway was not practicable. a) Onboard vessel STEFANI, stern: A labor was exposed to a fall of seven feet, eight inches while disembarking the vessel using a step ladder that was not secured to prevent movement and did not extend thirty six inches above the landing area. b) Onboard vessel BACCO, stern: A laborer was exposed to a fall of six feet while disembarking the vessel using a step ladder that was not secured to prevent movement and did not extend thirty six inches above the landing area.
