29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:        a)  Shop Area, 1 Anchor Way, Verona, PA 15147:  Observed on or about January 23, 2014, a Cincinnati brand 300 ton hydraulic press brake (model number 300H, serial number 30708) was not equipped with point-of-operation guards.  The press brake was operated by a foot pedal located on the floor.  Employees using the press brake to bend metal angles were exposed to a crushed-by hazard.     b)  Shop Area, 1 Anchor Way, Verona, PA 15147:  Observed on or about January 23, 2014, a Verson brand 25 ton press brake (serial number 14457-1062) was not equipped with point-of-operation guards.  The press brake was operated by a foot bar located on the front of the machine.  Employees using the press brake to bend metal bolts and plates were exposed to crushed-by hazards.

29 CFR 1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard(s):                                                                                                                                              a)  Shop Area, 1 Anchor Way, Verona, Pennsylvania 15147:  Observed on or about January 24, 2014, the tongue guards on both the left and right wheels of a Valley brand bench grinder (model number 171505) were missing.  The gap between the surface of the grinding wheel and the frame of the machine where the tongue guard should be was approximately 1 and 1/8" (left wheel) and 1 and 1/4" (right wheel).  Employees using the bench grinder to sharpen tools were exposed to struck-by hazards and fire hazards due to the missing tongue guards.

Employees using the bench grinder to sharpen metal tools were exposed to caught-by hazards.     b) Shop Area, 1 Anchor Way, Verona, Pennsylvania 15147:  Observed on or about January 24, 2014, the opening between the work rest and the abrasive wheel on a Delta Triple Duty Ball Bearing bench grinder was approximately 7/8" (left wheel) and 0.5" (right wheel).  Employees using the bench grinder to sharpen metal tools were exposed to caught-by hazards.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   a) Shop Area, 1 Anchor Way, Verona, Pennsylvania 15147:  Observed on or about January 24, 2014, the distance between the abrasive wheel and the adjustable tongue guard exceeded 1/4" on a Delta triple duty ball bearing grinder.  The distance measured between the surface of the grinding wheel and the bench grinder frame was approximately 1 and 1/4 inches (left wheel) and 1 inch (right wheel).  Employees using the bench grinder to sharpen metal tools were exposed to stuck-by hazards and fire hazards due to the improperly adjusted tongue guards.

The outlet box was not installed with proper strain relief for use as a pendent style outlet box.  Employees using the Cincinnati lathe were exposed to the hazard of electric shock and electrocution.         b) Shop Area, 1 Anchor Way, Verona, Pennsylvania 15147:  Observed on or about February 18, 2014, an outlet box being used to supply 220 Volt, 3 phase power to a H.E.S. 20" roll threader (model number 550, serial number 10898) was being used as pendent style outlet box. Employees were exposed to the hazards of electric shock and burns while changing the shear blades on the Niagara JL-8 shear.   b) Shop Area, left of door leading to back room, 1 Anchor Way, Verona, Pennsylvania 15147:  Observed on or about February 18, 2014, the cover on an outlet box was partially disconnected from the box due to missing screws to hold the cover in place.  Employees using any of the four available receptacles on the outlet box were exposed to the hazards of electric shock and burns.

Employees using the hot header machine were exposed to the hazard of electric shock. burns, and electrocution.       b) Shop area, 1 Anchor Way, Verona, Pennsylvania 15147:  Observed on or about February 18, 2014, a flexible cable was being used to supply power (220 volt, 3 phase) to a Cincinnati lathe (Cintilathe).  the cable was strung from the ceiling to the machine.
