29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: 1) The employer does not administer a continuing, effective hearing conservation program when employee noise exposures equal or exceed an 8-hour time-weighted average (TWA), or equivalent dose of fifty percent. This violation occurred on or about February 13, 2014 when employees were exposed to sound levels above eighty-five (85) dBA in the band saw area of the foundry. 2) The employer does not administer a continuing, effective hearing conservation program when employee noise exposures equal or exceed an 8-hour time-weighted average (TWA), or equivalent dose of fifty percent. This violation occurred on or about February 13, 2014 when employees were exposed to sound levels above eighty-five (85) dBA in the casting area of the foundry.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: 1) On or about 2/13/14, in the machine shop, the employer allowed an employee to dust off equipment using an air nozzle operating at 60 psi. 2) On or about 2/13/14, in the large-mold casting area of the foundry, the employer allowed an employee to dust off molds using an air nozzle operating at 35 psi. 3) On or about 2/13/14, in the large-mold casting area of the foundry, the employer allowed an employee to dust off molds using an air nozzle operating at 80 psi.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: 1) On or about 01/23/2014, in a central portion of the facility near the northwest most aluminum furnace, the employer allowed employees to work near a furnace motor with a damaged junction box in order to melt and pour aluminum. 2) On or about 01/23/2014, in a central portion of the facility near the southeast most furnace (used for brass), the employer allowed employees to work near a furnace motor with a missing juction box cover in order to melt and pour aluminum.
