On or about January 13, 2014, on top of the baffle box, employees installing the sunglide lid were not protected from a 15-foot fall hazard by use of a fall protection system. b. On or about January 10, 2014, on top of the 54-inch reinforced concrete pipe, two employees were not protected from a 7 feet fall hazard by use of a fall protection system.
