29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) Roof, new residential construction located at 3534 South Sherwood Road, Smyrna, GA 30082: Employees using positions belts and or front rings on the body harness on a 10/12 pitched roof at heights of approximately 21 feet used an inadequate fall arrest system when the attachment to the body harness was not made on the wearer's back. Employees attached life-lines to caribineers located in the front webbing of the body harness which was located at the wearer's waist. A positioning device can not be used as a personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Roof, new residential construction located at 3534 South Sherwood Road, Smyrna, GA 30082: Employees were not provided fall protection training. Employees attached life-lines to the roof trusses by wrapping it around the truss and tying a makeshift knot and not using the metal ridge anchor which is provided with the Guardian Fall Protection Roof Kit. Employees also were not connected to the D-Ring on the body harness as they were attached to the front positioning rings located on the front side of the harness. Employees were exposed to potential injuries and or falls while using this inadequate fall protection system.
