29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Facility located at 1504 Ralph David Abernathy Blvd, Atlanta, GA 30310- The employer had not established a Lockout Tag Out (LOTO) program consisting of energy control procedures, employee training and periodic inspections. Energy sources requiring LOTO at the facility include gravitational, mechanical, electrical, hydraulic, pneumatic and thermal. Employees were exposed to crush and struck by hazards while servicing a vehicle without the wheels chocked/blocked and hand brake engaged. Employees worked underneath the vehicle while it was lifted and supported by a service floor jack.                                                                                  AND/OR in the alternative   OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts:    a) An employee was servicing a 1999 Mazda 626 LX; VIN #: 1YVGF22C5X5885092 installing a transmission and components in the auto shop driveway. The employee was fatally injured when crushed between the ground and the under carriage of the vehicle.
