29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):     No lockout/tag out program was developed for machines including, but not limited to, band saw and CNC machines. These machines are powered by electric motors and the mechanical energy of motion can cause injury, if the machines start, while employees are handling the machine parts or tools.

29 CFR 1910.178(l)(1)(ii):  The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:     Employees who operate the fork truck to unload truck and move stock were not trained.
