Employees were exposed to struck-by hazards while setting up concrete forms for a sewer inlet in an excavation at least 11 feet deep. Loose chunks of soil larger than 6 inches in diameter were observed within 2 feet from the excavation face.     Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
