Trim Saw (circular table saw with a blade approx. 6 inches to 8 inches in diameter) was not guarded to prevent accidental contact during operation. Guard was not provided to automatically adjust to the thickness of the stock being trimmed. Observed on/or about 01/28/2014.

Chemicals include but not limited to Blanket Wash, Printing Ink, Soy Base Coating and Prisco Power Klene. Observed on/or about 01/28/2014.
