OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard:    a) Plant 9, Near Core Setter - The portable A-frame ladder being used to access the working level was not used as it was designed, exposing employees to a fall hazard.  The A-frame ladder was being used in a folded position and not fully opened, with spreaders locked and all feet contacting a firm level support surface, on or about January 28, 2014.    No abatement certification or documentation required.

29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.    a) Plant 3, Shot Blasting Area - Employees walking around the shot blasting machine are exposed to slip and fall hazards from steel shot on floors and in isles, on or about January 28, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
