29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service. Jobsite: Employees were using a 5 ton swivel hook with no safety latch attached to the suspended load exposing employees to potential rigging failure.

29 CFR 1926.652(c)(3)(iii): At least one copy of the tabulated data, which identifies the registered professional engineer who approved the data, shall be maintained at the jobsite during construction of the protective system. After that time the data may be stored off the jobsite, but a copy of the data shall be made available to the Secretary upon request. Jobsite: Employees on site were using two (2) rented trench boxes on site with no Tabulated Data available for review.
