At the south side of the building, employees were exposed to a fall hazard 13 feet 6 inches, in that, the second frame planking was extended to create a long platform and was rested at the middle section of the platform erected at the east side of the building.

At the site, employees were working on a two plank platform at the top frame of a scaffold while setting up for concrete pumping. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold structure, personnel hoist, or similar surface was not used: a. At the site, employees were exposed to a fall hazard while climbing the frames to access the outriggers and the top of a two frame scaffold, 15 frames wide, while setting up for concrete pumping.
