29 CFR 1910.132(a)(1)  . Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  a) Laundry, for employee(s) who changed out chemicals, including but not limited to corrosive Neutralizer approximately every 3-4 days, the goggles present were unsanitary for use.    a) Soiled Utility room 200 Hall, for employee(s) who rinsed out soiled linen in the hopper, the goggles present were unsanitary for use.

29 CFR 1910.1030(d)(3)(i):  When there is occupational exposure, the employer did not provide, at no cost to the employee, appropriate personal protective equipment such as, but not limited to, gloves, gowns, laboratory coats, face shields or masks and eye protection, and mouthpieces, resuscitation bags, pocket masks, or other ventilation devices. Personal protective equipment will be considered "appropriate" only if it does not permit blood or other potentially infectious materials to pass through to or reach the employee's work clothes, street clothes, undergarments, skin, eyes, mouth, or other mucous membranes under normal conditions of use and for the duration of time which the protective equipment will be used.  a) Soiled Utility room 200 Hall, no impervious gown or apron, or face shield was present for employee(s) who rinsed out soiled linen in the hopper.  The goggles available were dirty and needed replacement.  b) Soiled Utility room 100 Hall, the gown available for employee(s) who rinsed out soiled linen in the hopper was not impervious.  c) Soiled Utility room 300 Hall, the gown available for employee(s) who rinsed out soiled linen in the hopper was not impervious.

29 CFR 1910.305(g)(1)(iv)(B)  Unless specifically permitted in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used where run through holes in walls, ceilings, or floors:  Conference room outside the DON and Social Services Offices, a duplex outlet was servicing a flexible cord that ran through a hole in the ceiling to the time clock.
