29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Workplace, in warehouse- a DeWalt Palm Sander was plugged into a metal quad outlet box that was not mounted to a wall, beam, etc.. The quad box was not used in accordance with manufacturer instructions; on or about 1/28/14.    b) Workplace, in warehouse- a Global Electric Pedestal Fan was plugged into a metal quad outlet box that was not mounted to a wall, beam, etc.. The quad box was not used in accordance with manufacturer instructions; on or about 1/28/14.    c) Workplace, in warehouse- a dual receptacle outlet box was not mounted to a wall, beam etc.. The box was not used in accordance with manufacturer instructions; on or about 1/28/14.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:    a) Workplace, west wall of warehouse area- Breaker box used to provide power for workplace lights, fans and hand tools was blocked by two wooden tables. Tables had several pieces of unused fitness equipment on top of and one metal ladder leaning against them; on or about 1/28/14.    b) Workplace, west wall of warehouse area- Breaker box used to provide power for workplace lights, fans and hand tools was blocked by two wooden tables. Tables had several pieces of unused fitness equipment on top of and one metal ladder leaning against them; on or about 1/28/14.    c) Workplace, west wall of warehouse area- Breaker box used to provide power for workplace lights, fans and hand tools was blocked by two wooden tables. 29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    a) Warehouse, Technician's work area- a De Walt Palm Sander and a Cybex Treadmill were plugged into a pendant outlet that did not have a cover plate thereby exposing energized parts; on or about 1/28/14.    b) Warehouse, Technician's work area- a pendant outlet that did not have a cover plate was used throughout the day by Technician's plugging in their hand tools thereby exposing Technicians to energized parts; on or about 1/28/14    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
