29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a) Washington Ave. refrigerated warehouse, stairwell 5th, 4th, and 3rd floor: Stairwell was not kept clean in a sanitary condition free of hazards. Chunks of ice were frozen on the stairways and stair landings, which poses a slippery hazard to employees who use the stairwell frequently to move between floors and to escape in cases of emergency. On or about 01/28/14. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

29 CFR 1910.22(b)(2): Permanent aisles and passageways were not appropriately marked. a) Washington Ave. refrigerated warehouse, 1st, 2nd, 3rd, 4th, and 5th floor: Permanent aisles and passageways in the freezers were not marked. Employees used 37 inches wide forklifts to move items in aisles that were between 65 to 75 inches wide, and placed and stored items in aisle space which reduces clearance space for safely handling mechanical equipment. On or about 01/28/14.
