The employer did not develop, document and require employees to utilize a lockout procedure for the slitter that required employees to de-energize and lockout the slitter before performing setups when they were exposed to the moving, unguarded rotary slitter head at the point of operation. No other effective means of protection, such as machine guarding, was provided. There is no abatement documentation or certification required for this item.
