29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   a) Jobsite: 418 Federal street - The working level of a fabricated frame scaffold was not fully planked or decked between the front and rear uprights thus exposing employees to a fall hazard of approximately twenty-six (26) feet. Observed on or about 12/5/13.

Noted on or about 12/05/13.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) Jobsite: 418 Federal Street - Employees performing bricklaying operations from the 4th level of the fabricated frame scaffold were exposed to fall hazards of approximately twenty-six (26) feet without the use of personal fall arrest systems or guardrail systems. Observed on or about 12/05/13.   DD Fox Construction, LLC. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 315816264, citation number 1, item number 5a and was affirmed as a final order on 11/18/2011, with respect to a workplace located at 1345 S. 6th street, Philadelphia PA 19147.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     a) Jobsite: 148 Federal Street - A ladder or other safe means of access was not provided to employees performing stucco operations on a fabricated frame scaffold.  Employees were using the side frames and cross-braces as a means of access/egress, thus being exposed to fall hazards up to twenty-six (26) feet above ground level.  Observed on or about 12/05/13.   DD Fox Construction, LLC. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 315816264, citation number 1, item number 2 and was affirmed as a final order on 11/18/13, with respect to a workplace located at 1345 S. 6th Street, Philadelphia PA 19147.

Employees were performing bricklaying operations on the 4th working level, approximately twenty-six (26) feet above ground level. Observed on or about 12/05/13.
