OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards created by the use of industrial storage racks which were not anchored to the floor:            a) Building 16, Brick Saw Bay Area, 730 Superior Street, Carnegie, Pennsylvania, 15106.  Observed on or about January 30, 2014, employees were using industrial storage racks that were not anchored to the floor.  The industrial storage racks were used to store pallets of ceramic bricks and other materials.  The industrial storage racks were at risk of tipping over while being loaded or unloaded by forklifts.  Employees loading the industrial storage racks and employees working in the area of the industrial storage racks were exposed to crushed-by and struck-by hazards.       b)  Building 29, 730 Superior Street, Carnegie, Pennsylvania, 15106.  Observed on or about January 30, 2014, employees were using industrial storage racks which were not anchored to the floor.  The industrial storage racks were used to store various types of materials and equipment.  The industrial storage racks were located along the interior walls of Building 29.  Employees loading the industrial storage racks and employees working in the area of the industrial storage racks were exposed to crushed-by and struck-by hazards.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:     (a) Warehouse Building, 730 Superior Street, Carnegie, Pennsylvania 15106:  On or about January 30, 2014, no railing was provided along the open-sided floor approximately 4 feet 2 inches above the lower dock floor area.  Employee(s) retrieving materials from or placing materials into the warehouse were exposed to potential fall hazards created by the lack of guardrail around the open pit.

29 CFR 1910.303(g)(2)(i):  Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures:   a) Warehouse, northeast wall, 730 Superior Street, Carnegie, Pennsylvania 15106:  Observed on or about January 30, 2014, an electrical panel box was not effectively closed because the cover was missing.  Energized components inside the electric panel were exposed.  Employees using switches inside the electric panel box to operate ceiling lights in the warehouse were exposed to the hazard of electric shock and burns.   b) Warehouse, southeast wall, 730 Superior Street, Carnegie, Pennsylvania 15106:  Observed on or about January 30, 2014, an electrical junction box located directly above two Square D brand fuse boxes on an interior wall between the bay doors was not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures.  Conductors located inside the junction box were exposed due to a missing front cover on the junction box.  Employees using the bay door to access the warehouse were exposed to the hazard of electric shock and burns.       c) Warehouse, southeast wall, 730 Superior Street, Carnegie, Pennsylvania 15106:  Observed on or about January 30, 2014, a Square D brand fuse box (220 Volt) was open.  The fuse box could not be closed due to a yellow rope which was tied to components within the fuse box.  Employees accessing the warehouse through the bay door were exposed to the hazard of electric shock and burns.   d) Main Shop Area, east wall, brick saw bay area, Building 16, 730 Superior Street, Carnegie, Pennsylvania 15106:  Observed on or about January 30, 2014, an electrical junction box located beneath a Square D brand safety switch labeled "Single Phase 240 with neutral A" (yellow making/hand written) was not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures.  The front of the junction box was open and the conductors inside were exposed.  Employees working in the area of the industrial storage racks were exposed to the hazard of electric shock and fire.    e) Warehouse, southeast wall, 730 Superior Street, Carnegie, Pennsylvania 15106:  Observed on or about January 30, 2014, the metal box housing the bay door switch was broken.  Employees using the bay door switch were exposed to the hazard of electric shock and burns.

29 CFR 1910.178(a)(6): Nameplates or markings for powered industrial trucks were not in place:                                                                                                                                       a) Main Shop Area, 730 Superior Street, Carnegie, Pennsylvania 15106: Observed on or about January 30, 2014, the employer did not ensure all nameplates and markings were in place on an orange LPG Toyota brand forklift (unknown model or serial number).  The forklift was used to move materials between the yard, warehouse, and buildings 16 and 29.  There were no markings on the forklift to identify the model number, serial number, rated capacity and information.  Employees using the forklift to move materials throughout the facility were exposed to struck-by hazards.
