29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. A.) Erie Contract Interiors, Inc., 3700 Route 957, Russell, PA 16345, middle / high school renovation, on or about January 16, 2014 and times prior thereto: Employees were cutting carpeting and tile with various hand cutting tools, including, but not limited to, utility knives, seam trimmers and carpet knives. The employees were not provided with gloves to prevent injury (e.g. lacerations) to the hands when working with these tools.

Erie Contract Interiors, Inc., 3700 Route 957, Russell, PA 16345, middle / high school renovation, on or about January 16, 2014 and times prior thereto: Employees were utilizing personal protective equipment that included but was not limited to safety glasses, hard hats, and protective footwear. Personnel had not received personnel protective equipment training on topics including, but not limited to, the maintenance, limitations, care and storage of the personal protective equipment.
